Welcome as a patient at Q KLINIK. In connection with your treatments at our clinics in Frederiksberg and Østerbro, it is necessary for us to record and process relevant information about you and your health.
WHAT PERSONAL INFORMATION DO WE STORE?
During your treatment at the clinic, we record information about your health and medical history for the purpose of maintaining a medical record, as required by current legislation for authorized healthcare professionals. The specific rules are found in Chapter 6 of the Authorization Act and in the Medical Record Keeping Order.

In addition to the health information we record about you, we may need to request/receive additional health information important for your current treatment from other authorized healthcare professionals, including your GP or a specialist, provided you give consent. This could include test results or information about other health conditions, as described in Chapter 9 of the Health Act.

The information is solely used to ensure good and safe treatment for you, as well as the associated administrative functions.
We are obligated to store your information securely and confidentially. At Q KLINIK, we use a web-based clinic system, ComplimentaWork, specifically developed for the healthcare sector and compliant with the requirements for storing and handling sensitive personal information.
We may also record other information about you for billing purposes. These details are recorded in accordance with the Data Protection Act. We may also use your contact information to send newsletters or other marketing initiatives, but only if we first obtain your consent.

SHARING OF PERSONAL INFORMATION
Under the Health Act, we are obligated to maintain confidentiality regarding your health and other sensitive information. However, if necessary, we may share your health information internally among our staff. Sharing health information outside the clinic generally requires your consent. In special cases, information may be shared without consent, in accordance with the Health Act, typically with other healthcare professionals, such as your GP. The rules are outlined in Chapter 9 of the Health Act.

Information recorded for billing purposes may be shared with payment intermediaries to facilitate transactions. If your treatment is paid for in part or in full by someone other than yourself, such as an insurance company, we will also share necessary treatment information with the payer. An insurance company may require information about the areas treated in connection with reimbursement for your treatment.
HOW LONG DO WE RETAIN PERSONAL INFORMATION?
Your medical record is stored according to regulations established by the Danish Patient Safety Authority. The current retention period is at least 5 years, as stated in §15, section 2 of the Medical Record Keeping Order.

In certain cases, medical records may be stored longer.
Information used for billing purposes is kept for as long as necessary for accounting and bookkeeping.
Email correspondence between you and the clinic is deleted when no longer relevant, and at the latest after 1 year.
YOUR RIGHTS REGARDING PERSONAL INFORMATION
You have the right to request access to the information we have registered about you by contacting us. Under §24 of the Authorization Act, we are not permitted to delete information from your medical record, but if you believe there are errors in the record, you may request an amendment.

Incorrect information not covered by current laws, including the Health Act, Authorization Act, and laws described in the Medical Record Keeping Order, can be corrected or deleted upon request. You also have the right to request that we stop processing such information.
Once you turn 15, you generally have the right to know what is recorded about you in a medical record within the healthcare system.
COMPLAINTS ABOUT HANDLING OF PERSONAL INFORMATION
Complaints about our handling of your personal information can be submitted to the Danish Data Protection Agency, Borgergade 28, 5th floor, 1300 Copenhagen K. You can find more information about the Data Protection Agency at www.datatilsynet.dk

UNINTENDED EVENTS
An unintended event is an incident that causes harm or risk of harm. An unintended event can be identified by the thought: “This shouldn’t have happened. It poses a risk or affects my or my relative’s treatment. I want to make sure that professionals learn from this mistake, both at the current treatment location and in the healthcare system in general.” The event is unintended because the professionals involved did not intend to cause harm.

PATIENT COMPENSATION
The Patient Compensation Association processes claims from patients who are injured in connection with treatment at a hospital, by a GP, or as a result of side effects from a medication.
The Patient Compensation Association compensates for injuries and deaths resulting from treatment, lack of treatment, or delayed treatment. They also compensate for harmful side effects caused by medications. Compensation is provided for financial losses, pain and suffering, and/or permanent injury.
